Please follow the instructions below to prepare for the new dental experience at Mill Pond Dental Care.  

 

How to Prepare and Check in For Your Dental Appointment

STEP 1.

Prepare at Home

Review the Patient Information and Appointment Guidelines document provided below.

Complete and submit the Digital Patient Screening Form.


If you do not submit the Patient Screening Form electronically, Please print and fill out the Printable Version and bring it with you to your appointment. 

Review the Virtual Waiting Room Instructions referenced below in Step 2.

Please fill out this form and submit online prior to you dental appointment

If you are not able to submit electronically, please download the Printable Version, fill it out and bring it with you to your appointment

 

Step 2. 

Check in

Go to Mill Pond Dental Care for your scheduled appointment.

Please bring a mask with you.

Please bring your filled out Patient Screening Form if you did not submit one electronically. 

Please arrive 10-15 minutes before your scheduled appointment to allow time for screening. 

Please stay in your car when you arrive. Please do not go into the building. 

When you are parked in the Mill Pond Dental Care Parking lot, please click on the link below to enter the Virtual Waiting Room. The Virtual Waiting Room is how we prefer to communicate with patients who have arrived. 

Once you have entered the Virtual Waiting Room, please keep your screen active and please do not navigate away from the Virtual Waiting Room screen until we acknowledge your arrival.

If you are not able to enter the Virtual Waiting Room, please call 519​-268-3351 to check in for your appointment.


Please be prepared to wait in your car or outside until we are able to invite you into the building.
