Review the Patient Information and Appointment Guidelines document provided below.
Prior to your appointment you will receive a text message or e-mail with a link to documents that will need to be completed and submitted. This link will give you access to our COVID-19 Screening Form. You may also fill out this form ahead of time by clicking on the link below.
If you do not submit the Patient Screening Form electronically, we will help you to complete the form on our tablet upon arrival.

Go to Mill Pond Dental Care for your scheduled appointment.
Please bring a mask with you.
Dentistry is a health care setting. Masks are still required in health care settings.
Please arrive 10-15 minutes before your scheduled appointment to allow time for screening.
Please stay in your car when you arrive. Please do not go into the building.
